Here's the plain-English version. One of the least-talked-about perimenopause symptoms is suddenly-achy joints. You wake up and your hands feel stiff. Your knees complain going down stairs. Your shoulder catches. And you didn't do anything to cause it.
Estrogen has a quiet anti-inflammatory effect throughout your body. It also helps maintain cartilage and joint lubrication. When it drops or swings, inflammation rises and joints feel it first.
Where it usually shows up
- Hands and fingers — morning stiffness, sore knuckles
- Knees — aching going up or down stairs
- Hips — dull ache, especially at night
- Shoulders — "frozen shoulder" is far more common in perimenopause
- Feet, especially first steps in the morning
What helps
- HRT. Joint pain is often one of the first things to improve when women start estradiol. For some it's dramatic.
- Omega-3. Real evidence for reducing joint inflammation.
- Strength training. Counterintuitive but true — stronger muscles around a joint reduce pain more than rest does.
- Walking daily. Joints need movement to stay lubricated.
- Losing 5–10 lbs if you're carrying extra weight. Each pound off your body is 4 pounds off your knees.
When it's something else
If a single joint is red, hot, visibly swollen, or the pain is severe and one-sided, it's not perimenopause — get it looked at. Perimenopause joint pain is usually symmetric and mild-to-moderate, across multiple joints, and comes and goes with hormonal swings.
